Integrated Services Case Manager - Kaiwhakaoranga Specialist Case Management Service - Christchurch

 

12 month secondment / fixed term opportunity 
 

Based in Christchurch, CBD – with regional travel required

We have an opportunity for an Integrated Service Case Manager (ISCM) to join our  Kaiwhakaoranga Specialist Case Management Service.  This is a multi-disciplinary team delivering a comprehensive, holistic and culturally responsive service to the affected whānau, survivors and witnesses of the 15 March 2019 terrorist attack, and their wider whānau.  

As an ISCM you will understand the needs of individuals and whānau using a holistic framework. This may include health, social, financial, immigration, education, children, housing and other needs as they arise.  You will be able to build a relationship of trust and understanding to uncover the needs and then link whānau to appropriate wrap around services to meet their needs. You have an empathetic, open and culturally responsive approach. 

Experience navigating health, mental health or social services is highly advantageous in this role, as is an understanding of Islamic principles and faith. 

 

To be successful you need:

  • to be able to understand complex situations and dig deep to find the root cause 
  • navigate services and find creative solutions to meet the needs of your clients 
  • empathy, care and understanding
  • experience creating action plans and tracking client progress with the ability to motivate people to achieve their goals
  • high levels of integrity and the ability to quickly build trust with others
  • good computer literacy and web navigation skills (research to find solutions)
  • to be organised and write quality notes 
  • to be able to travel regionally as required – driver's licence is essential.

The remuneration for this role is between $71,231 to $87,060 per annum.  Application along this salary scale is dependent on relevant work experience, evidenced skills and other factors.
