About The Role

You will ensure that Residents admitted to the Care Home are cared for at the appropriate level of care and will support the Care Home with timely and efficient discharge of the Resident.

For you, that will mean taking responsibility for a specific part of the home and delivering the very best level of care. So, whether you’re reporting that the new lady who just came in appears to have some mobility issues, asking a gentleman to provide a sample for monitoring or administering a medicine to someone, you’ll always have resident dignity and wellbeing in mind above all. Within this role you will provide intermediate care.

The working hours will be 42 hours, 5 days over 7 – weekend work is essential in this role.

About You

 The Admission & Discharge Coordinator can be a Registered Nurse with a valid PIN / Senior Carer with Level 3 in Health & Social Care including Supervision & Medication Training.  You will have proven leadership skills in a care home for vulnerable adults.

Beyond that, it’s all about your brilliant, positive energy and natural ability to get along with people. This is a role for someone who’s ready to take real accountability. And, whenever you communicate with a resident or co-worker, you’ll be truly considerate and kind.
