The National Contact Centre is a first point of contact for requests for transition support where the delivery of services and support would be through a variety of channels, including calls, electronic response (Email, Text, Live Chat) and social media.
They will work in collaboration with other transition professionals to help produce effective outcomes for young people.

This new initiative works within the National Contact Centre Transition Advice & Assistance team to support young people to understand their entitlements, be connected to the services and supports available to them, provide financial assistance where appropriate and to put them in touch with face-to-face support should they require it.
Transition Advisors need to be able to engage and build a relationship with rangatahi, responding to their needs and supporting them in their challenges.
You will respond to requests for advice and assistance from young people and young adults that are transitioning from care and youth justice to adulthood (ages 15 - 25 years).
At times this may involve responding to requests from other people supporting these young people, such as whÄnau, caregivers or other professionals.
