Utilize your experience and knowledge as a Support Worker to enable the person you will be supporting to live in their own homes and connect with their community.

 About the role

You will support a 42 year old woman living in her own home.  History of trauma and abuse, lives with a high level of anxiety.  Needs a very consistent approach to her care and significant daily reassurance.

 And a 41 year old male (transgender) with cerebral palsy and non-verbal. Requires assistance with personal cares.  You will report to Andrea Bowness and have interaction with the family about the support being provided. They love crafts and gentle exercise.

The job requires you to work 80 hours fort night.

Duties of the role

• Support the individual to achieve their personal plans.
• Strengthen and develop the individuals networks in the community.
• Engage with the individual to check that the support being offered is meeting expectations and enhancing their life.
• Support the individual with personal hygiene and wellbeing.
• Promote an environment that encourages the individual to feel safe and comfortable.
• Encourage the individual to grow and increase their independence by supporting them to learn skills for daily living and to make decisions and choices.
• Generate daily support notes, incident reports and daily handover notes to your co-workers.
• Collaborate with other support staff by attending staff meetings, coaching, keeping each other informed about the individual being supported and sharing ideas with your team members.

Skills and experience

• It is essential that you are fully vaccinated (mandated)
• Qualification in Health and Wellbeing or similar OR two years experience
• Computer literacy 
• Valid drivers license essential

• Able to follow a clear behavior plan

• Comfortable managing personal care support
• Available for all shifts (mornings/afternoons/wake overs/weekends)
