An NDIS Support Coordinator is required to join a team of support workers and specialists who together aim to empower participants to live independently.
We are open to leaders new to NDIS that would like to grow in the industry!
The Role:
As a support coordinator you will hold the below duties and responsibilities:
Lead and Mentor support workers.
Empower Participants to understand, implement and review their NDIS plans
Allow participants to exercise choice and control over the supports received to assist the individual in achieving their goals
Educate participants, families, guardians, support staff and care givers about participant goals, care plans and service options
Accurately document all communication with all parties involved
Review health care plans, clinical assessments and allied health reports to fulfill NDIS requirements in order to achieve the best possible outcome for participants
Collaborate and contribute to NDIS pre-planning reports for team review with management prior to NDIA planning meetings
About You:
To be a strong applicant for this role, you will:
Have experience within leadership – We are open to entry level leaders new to NDIS.
Are able to communicate strongly and have the ability to maintain working relationships with all stakeholders
Have a deep understanding of individuals living with disabilities and their needs, as well as their caregiver needs
Demonstrate an understanding of service and related systems for individuals with psychological and other disabilities
Believe in a strong work ethic and promote positive experiences at work
