Position Summary:
The Commonwealth Home Support Programme (CHSP) Community Nursing Service provides clinical nursing care to clients who live and/or work in the Local Government Areas of Waverley, Woollahra, and the City of Sydney. Nursing care is delivered in the community or clinic setting, by Registered Nurses, and is generally short term and episodic in nature. The purpose of this intervention is to optimise the independence and function of CHSP eligible clients in order for them to continue living in their home environment, reduce hospital length of stay and avoid unnecessary hospital presentations.
This exciting position involves the Registered Nurse providing specialist nursing services to community based aged care clients and their carers including the provision of client centred, evidence based nursing care and is expected to embrace practice development principles in the delivery of that care. Care will be provided from a quality framework and using a re-ablement approach that promotes positive health outcomes and client independence.

The vacancy is open to Australian citizens, New Zealand citizens who hold a valid New Zealand passport, Australian permanent residents and those non-residents deemed by the Department of Immigration and Citizenship to be eligible to work in Australia only.

Selection Criteria:
1. Personal integrity and demonstrated commitment to the Philosophy, Mission and Values of Mary Aikenhead Ministries and St Vincent’s Health Australia.
2. Currently registered as a Registered Nurse with the Australian Health Professionals Regulation Agency and Current NSW drivers license (Class C or above) in order to attend client’s home with provided car.
3. Demonstrated ability to work within an interdisciplinary team.
4. Demonstrated computer literacy with email and an electronic patient record system
5. Demonstrated effective communication and interpersonal skills.\
6. Demonstrated effective time management skills and ability to prioritise competing workloads with in a complex environment.
7. Demonstrated customer service and patient centered approach to care.
8. Experience working within Aged Care and/or community healthcare setting is highly desired.
