Position Profile: This position will be responsible for providing nursing interventions and identifying patients who could avoid admission to hospital with the provision of nursing intervention at home. Care can be provided either in the in the patient’s home or through advice and support to GP’s/Community organisations as a member of the Hospital Avoidance Program’s Hospital Admission Avoidance (HAAV) Team. HAAV is a multidisciplinary service that works collaboratively to identify eligible patients within the ED and directs these patients to appropriate nursing and/or allied health in the home or community-based services. HAAV also refers to the Hospital Avoidance Program’s (HAP) Rapid Access Medical (RAM) clinic and other specialist outpatient clinics as required. The HAAV and the RAM Clinic are the two arms of the HAP and work together to provide alternative pathways to hospital admission.
