We have an exciting opportunity for a full time, permanent HealthCare Assistant to join our Southern Community Health Team

The Franklin Community Health Care team services the based at the Pukekohe Hospital Site is part of Counties Manukau District Health Board. We are part of the Southern Community Health Team and work closely with the Manukau Team, based at Papakura. This interdisciplinary team provides assessment and treatment for clients in their own homes.

The vital role of a HealthCare Assistant is to provide community based services that support patients to remain in their own homes. Healthcare Assistants deliver appropriately delegated health interventions under the supervision of a clinician.  Tasks include pressure area care, support to take medications (prompting), assist and support with home exercise programme, set up equipment and educate/train patient, stoma and catheter care and simple wound care.  Another part of the role will include supporting our new re-ablement programme.

You will be required to work primarily out of the Franklin Community Health Care base and occasionally, required to work across the other Home Health Care bases.

The ideal candidate for this role will be an adaptable, flexible, able to use own initiative and professional with a ‘can do’ attitude.

You will be given a comprehensive orientation, and opportunities for on-going in-service education.

The Southern Community Health Team provides services 7 days a week.

Essential Requirements:  

• Health Care Assistant Level 4 certificate
• Full & Clean NZ Driver's Licence
• Community experience (preferred)
