Sydney Local Health District (SLHD) is seeking a dynamic Community Mental Health Clinician to join our CORE Team at Canterbury Hospital!
Do you have relevant qualifications, and current registration with AHPRA or eligibility for membership to relevant professional body?
Do you have competencies to provide contemporary mental health practice, with a current NSW driver's licence?
The Community Mental Health/Core Team model of care states:
The “Community Mental Health Team†(Core Team) is the mainstay of the community mental health system. Core Teams and provide a range of clinical services including intake, assessment, consultation, treatment, and care coordination. Care coordination is the main component of this model of care and includes extended care coordination, targeted packages of care, and short term care coordination.
Consumers under the care of Core Teams will either have: * Short term interventions and may be discharged or referred back to their GPs or other treatment providers after a period of weeks or months.
The Mental Health Clinician will practice in keeping with the standardised Sydney LHD position description for the profession in addition to this position description.
