What you’ll be doing

The position of Clinical Psychologist, Care Coordination Team exists within Central Coast Local Health District, within the Mental Health Service. The position is based in Community Health Centres on the Central Coast. The Care Coordination Team provides non-acute assessment, clinical follow up and treatment (group and individual), care planning, and case management, for clients presenting with high moderate to severe mental illness including those clients requiring treatment under the NSW Mental Health Act 2007. The Team liaise closely with local General Practitioners and other service providers to assist clients to develop sustainable support networks and promote positive mental health outcomes. The position sits within the multidisciplinary team comprising Psychology, Social Work and Nursing. All aspects of service design and delivery are recovery oriented and therefore client-centred and collaborative, family inclusive and trauma-informed. The recovery model assumes that people with complex mental health conditions have the capacity to improve and to live a life that is not defined by their illness.

All Psychologists employed within the CCLHD are members of the Psychology Department. The Department monitors adherence to the professional code of conduct, promotes excellence and provides opportunities for ongoing professional development.
