About Us:

Sacred Heart Palliative Care Service is a growing service, offering excellent opportunities for professional support and development. About You:

You will be expected to provide evidence-based specialist palliative nursing care to people of all ages and cultural groups residing in the community setting. Adopting a leadership role in the coordination of nursing and health care within the palliative care service at Sacred Heart Hospital, you will be pivotal in facilitating optimal health outcomes for patients and their families. It is envisaged that the you will work closely with medical teams, offering consultancy, education and support to professional and non-professional colleagues. You will also contribute to service planning, audit and research processes.

About the Role:

The position is a permanent, full time position working within the Sacred Heart Health Service. The position will be required to deliver specialist palliative care intervention to patients living in the scenic Local Government Areas of Waverly, Woollahra, Randwick, eastern parts of Bayside and City of Sydney. Currently, the service is operating with a proportion of telehealth; utilising video platforms for some meetings and patient consultations.

Selection Criteria:

1. Personal integrity and demonstrated commitment to the Philosophy, Mission and Values of Mary Aikenhead Ministries and St Vincent’s Health Australia

2. Currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency and also an unrestricted NSW Class C Driver’s License.

3. At least 5 years full time equivalent post registration experience and in addition approved post registration nursing qualifications relevant to Palliative Care or such other qualifications or experience deemed appropriate by St Vincent’s Hospital, Sydney, with particular interest in aged care and palliative care.

4. Demonstrated ability to work autonomously and within the interdisciplinary team and demonstrated commitment to ongoing professional development.

5. Demonstrated skills in workforce education needs assessment with the delivery of tailored education programs to suit a range of adult learners within the community and sub-acute setting.

6. Demonstrated high level effective communication and interpersonal skills.

7. Demonstrated evidence of integration of theoretical knowledge and clinical skills in aged care and end of life scenarios, critical thinking and analysis in their daily practice.

8. Demonstrated effective time management skills and ability to prioritise competing workloads with in a complex environment.
