We are looking for a dynamic Clinical Care Manager with demonstrated experience in managing Care plans and Home Care packages, liaising with residents and families whilst overseeing and mentoring the care team.

This private facility is looking for a CCM to join their team alongside another CCM for their home North of Melbournes CBD by Bendigo.

This service strives for the best care and service delivery daily and have one of the highest ACFi ratings in the region.  

The successful candidate must be able to demonstrate:

Proven knowledge of best practice in an aged care or community setting and solid understanding of Aged Care Home packages.

Be able to promote a positive culture that embraces the values of this provider

Ensure compliance and adherence to policies and legislation.

Liaise with other allied health consultants and Medical staff

Carry out timely assessments and develop Care Plans for existing, new residents and those coming in for respite, to ensure it is a positive transition and a warm and welcoming experience.
