About the role

Clinical Care Coordinator 

Our client is looking to appoint a Clinical Care Coordinator to their aged care facility based in Mildura. 

Key responsibilities include: 

Lead the accurate and timely completion of reporting and documentation in line with relevant legislation, regulatory requirements, professional standards, policies, and procedures

Document care plans and clinical information

Drive continuous improvement and risk management

Provide people centred leadership and professional role modelling

Educate, support, and provide training

Manage assessments, reviews, and submissions

Ensure the facility functions in line with Residential Aged Care quality standards, legislation, and best practice guidelines

To be successful in the role you will have:

Bachelor of Nursing

Current AHPRA registration

Demonstrated experience in a similar role within aged care

Knowledge of relevant legislation, regulatory requirements, professional standards, policies and procedures

Strong written and verbal communication skills

National Police and NDIS clearances

Flu vaccination and Covid vaccinations

Registered Nurses

Key responsibilities include: 

Complete reporting and documentation in line with relevant legislation, regulatory requirements, professional standards, policies and procedures

Provide supervision and leadership to a team of healthcare professionals

Deliver specialised nursing care

Administer medications

Monitor clinical care plans

Participate in clinical auditing

To be successful in the role you will have:

Bachelor of Nursing

Current AHPRA registration

National Police and NDIS clearances

Flu vaccination and Covid vaccinations
