North Richmond Community Health (NRCH) is committed to making healthcare more accessible and culturally relevant. NRCH provides a range of medical, allied health, dental and community services to clients from all backgrounds.

The Care Coordinator role (full-time or part-time, fixed-term) is responsible for clinical leadership and care and wrap around service coordination for AOD and of the Medically Supervised Injecting Room (MSIR) clients.

Roles and responsibilities include, but are not limited to:

• Lead and drive client services to ensure a holistic approach to client health and wellbeing, service integration, care coordination and partnership development to provide the right services at the right time to meet identified client needs
• Develop and monitor relevant pathways and protocols to ensure the provision of care planning is available for all clients
• Participate in the development and maintenance of innovative service delivery models, that are flexible and adaptable to meet changing consumer needs
• Provide support and inputs for client care plans.
• Advocate widely for key issues that affect the people who inject drugs (PWID) community and the AOD program and keep abreast of latest developments within the AOD sector
• Participate in delivering education sessions on AOD tailored to the wider community
• Co-ordinate regular client health checks with internal and external programs

KEY SELECTION CRITERIA

Essential qualifications, attributes and skills include, but are not limited to:

• Qualifications in Health – Social Work, Nursing, Psychology or other is essential
• Minimum 5 years’ experience in providing AOD and/or mental health assessment and treatment services
• Demonstrated knowledge of care coordination for clients and experience in working in multi-disciplinary teams for provision of warp around services.
• Highly developed relationship management and negotiation skills with a broad range of stakeholders
• Demonstrated experience in negotiating service models with both government and non-government agencies
• Knowledge of local agencies and services relevant to the client group
• Experience working with people who inject drugs is highly desirable.
• Experience in quality management and accreditation processes.
