We have recently expanded Advancing Wellness at Home Initiative (AWHI) which is CCDHB's Early Supported Discharge service therefore we have an exciting opportunity to add to our Allied Health Assistant / Kaiawhina Hauora roles. Kaiawhina Hauora in AWHI deliver delegated activities in an interprofessional model to deliver reablement and promote self-management of health for the people we work with in their own home.
The main focus of AWHI is to support people's reablement or maintain functional levels, for those on a declining health journey, after a hospital admission. We identify and effectively support people's needs on discharge from hospital and facilitate that to happen as early as possible providing system benefits to wider hospital flow.
AWHI works with people in their own home immediately after discharge, from either of our 2 hospital sites, that have been medically unwell, sustained a significant injury or had a Stroke. The team is split over 2 bases, Ewart at Wellington Hospital site for the southern base and Kenepuru Hospital for the northern base, This role is based at Ewart. The 2 parts of the team cover all of CCDHB's catchment area between them, Seatoun up to Peka Peka but each team member has an identified base.

We offer on-going professional development opportunities including interporfesional support and training with the team of approximately 24 FTE around you and clinical supervision.
